Liquor Licenses
CITY OF PALMDALE
38300 Sierra Highway
Palmdale, CA 93550

January 3, 2010

Dear City Of Palmdale.

I am looking to see what happened to my liquor license application for my comedy club: SOMETHING SMELLS FUNNY

I sent in the paperwork and $92.00. I have not heard anything. SOMETHING SMELLS FUNNY is a 72 table comedy club in your city that is opening very soon. We will have liquor, beer, wine, fizzies, chugalugs, happy caps, & our signature 72 ounce Guzzler.

All papers are stamped and my check has been cashed. Papers were filled out by our manager Tito Laguna. Skip Fidget is our opening headliner. Followed by Mop Head. They have already been paid and I have over 2,000 t-shirts just waiting for my liquor license

I was told to write you with this information when I called.

Thank you for your assistance

Respectfully,

F. D. Nancy

19 Apr 2009

Dear Phoenix Hospital,

I was wondering if you could send me my medial records after treatment at your hospital while visiting Phoenix.

I was there from Feb 2, 2009 thru Feb 7, 2009. for hospital stay. I had a ladder removed from my head. I was in a construction accident and was impaled. This is a simple 4 step kitchen ladder with rubber on each step. It was in my head.

I now need my medical records for insurance purposes. Can you send?

I believe my doctors name was Dr Marteen Gui or Dr. Marvin Goo or something like that. (An outstanding emergency room physician!)

Your hospital gave me wonderful treatment I am recovering nicely. Although my ear continues to flap over (will not stay up, only 1 ear)

I need these medical records for my Insurance Company to properly assist me.

Thank you for your help and helping others. I am grateful.

Respectfully,

F. D. Nancy (MR)
